Accessibility is not a yes/no label. The useful questions are parking distance, grade, surface, restrooms, shade, seating, transfer requirements and what happens when a riverbank or trail gets wet.
Start with developed public sites.
Guadalupe River State Park notes accessibility resources including an all-terrain wheelchair by arrangement. For lake access, check individual facilities rather than assuming every Corps or county site has the same infrastructure.
